He is tired of fighting for money and fame. He wants peace. He dreams of returning to his hometown called Chiang Dao. It is a quiet place surrounded by mountains and nature. He wants to live there with his girlfriend Rin.

Rin is a doctor. She works in a large hospital in Bangkok. She is kind and brave. She believes in helping people no matter how difficult the situation is. She and Singh love each other deeply. They often talk about leaving the city and starting a simple life together in Chiang Dao.

Before he can leave, Singh must complete one final mission. He now works in security operations connected to VS Corporation. His superior officer is a man named Kriang. Kriang is serious and loyal to the company. He gives Singh the task of traveling to Bangkok to deliver special containers. Inside the containers are fish recovered from a sunken freighter in the ocean. The fish are believed to be a new food source that can help feed the population.

Singh and Kriang do not know that the fish are contaminated. When the freighter sank, it had entered waters where ancient bacteria were released from melting ice. The fish absorbed an unknown pathogen. This pathogen is powerful and dangerous.

When the containers arrive at VS Corporation, a senior official inspects the fish. Soon after, he begins to feel sick. His body becomes weak. His skin turns pale. He is rushed to the hospital where Rin works. Doctors try to treat him, but they do not understand what is happening. Within hours, the official dies.

But death is not the end.

Suddenly, the man opens his eyes. He moves in a strange and violent way. He attacks a nurse and bites her. Chaos begins. The infection spreads quickly. Patients and hospital staff are attacked. People scream and run through the halls. The hospital goes into lockdown. Doors are sealed. No one is allowed to leave.

News of the emergency is announced over the hospital speakers. Singh hears about the lockdown and the infection. He realizes Rin is inside. Without waiting for permission, he sneaks into the hospital. He is determined to find her and protect her.

The military has a secret plan. After rescuing Vasu and Anne, they will blow up the hospital. Explosives have already been placed in the building. They believe destroying the hospital is the only way to stop the outbreak from spreading to the city.

Rin learns about Vasu’s location and goes to his secure room. She hopes he can help her escape. Vasu promises to protect her if she continues to care for Anne. Rin agrees because she wants to survive. But when the commandos arrive, they bring their own medic and ignore Rin. Vasu chooses his own safety and abandons her.

At the same time, Singh and Buddy finally reach the neonatal ward. They find Meena. She looks tired and weak. She tells Singh that Rin was last seen near the upper floors. Meena hugs Buddy tightly. She tells Singh to take care of her son. Singh promises he will.

As Singh and Buddy leave, Buddy tries to run back to his mother. But Meena stops him. She begins to shake. Her eyes change. She has been infected. In front of them, she turns into one of the undead. In a heartbreaking moment, she attacks the infants in the ward. Singh pulls Buddy away. They are forced to leave her behind.

Buddy cries, but Singh tells him they must survive. The hospital becomes more dangerous. Zombies move in large groups. Gunfire echoes from different floors as commandos fight their way through.

Rin, abandoned by Vasu, is trapped in a hallway surrounded by zombies. Just when she thinks she will die, Singh appears. He fights fiercely, using his fists, elbows, and knees. He rescues her. They hold each other for a brief moment. Even in chaos, their love is strong.

They try to escape together with Buddy. But commandos spot them. Believing they are infected, the soldiers shoot. Singh is hit and falls. The commandos grab Rin because their medic has been killed by zombies. They need a doctor.

On their way, they see Vasu and Anne. Both have been bitten during a zombie attack. Vasu is in shock. He believes Anne has woken from her coma. But she is infected. Zombies surround them. Vasu chooses to stay with his wife and allows himself to be taken.

Singh, Rin, and Buddy reach the helipad. A helicopter with remaining commandos waits. Singh knows the zombies are close. He decides to stay behind and lead them away. He runs toward the hospital car park, shouting to attract them.

He hides inside a car, but zombies surround him. With no other choice, he detonates an explosive. The blast destroys many zombies. Flames fill the parking area.

Singh survives the explosion and runs back toward the helipad. But he sees more zombies climbing toward the roof. He realizes if he goes with Rin, the zombies will follow and reach the helicopter.

He makes one final decision. He tells Rin he loves her. He tells Buddy to protect her. Then he turns and fights the zombies alone. Rin and Buddy are pulled into the helicopter by the soldiers.

As the helicopter lifts into the sky, Rin watches through tears. She sees Singh fighting bravely against a sea of undead. Then the explosives planted in the hospital explode. The building collapses in fire and smoke.
